Trans men

Trans men will often prioritise having testosterone therapy and chest surgery, as these produce effects that are more immediately socially visible and aid day-to-day living in their reassigned gender role. Chest surgery, often called ‘top surgery’, is technically referred to as bilateral mastectomy with male chest reconstruction. Prior to surgery, a number of clients are likely to have bound their breasts in order to reduce their likelihood of automatically being gendered as female in social spaces. For many natal women with larger breasts that can’t simply be hidden by using layers of looser clothing, a binder can help to flatten chests and give them the appearance of a male chest.

Binding, although helpful in the short term, can be extremely uncomfortable, causing difficulty in breathing, back pain, and bruising to the ribs. Although clinicians and surgeons might sensitively talk through the physical aspects and impacts of chest surgery, you may be one of the few people whom your client can talk to in order to unpack the emotional aspects of surgery. Clients may have to wait a considerable amount of time for referral for top surgery and then for a surgical date. Helping them to process feelings of helplessness and anger can be a key part of being with your client through the process of transition.
